What happens when you fall backwards down the stairs?

As an EMT instructor, I tell trainees to consider the mechanism of injury en route to the scene. In this case we would want to know how many stairs (from what height), whether there were objects on the stairs (toys, clothing, etc), how big is the patient, what surface was the landing (carpet, tile, cement, wood), and how much time had passed since the fall.

Once on scene, I would look to make sure the patient's airway is clear and breathing is patent--if they bit their tongue, it could be bleeding and swelling. Then I would determine if he or she had head or neck trauma, followed by trauma to the chest, abdominal/pelvis, and extremities areas. Even if there is no visible wound, bruising, or swelling, I would suspect head/neck trauma and immobilize the neck with a cervical collar until the patient could be assessed by a physician.

I have seen patients with contusions and concussions, broken extremities, broken pelvis, broken necks. Others report whiplash lasting from weeks to months.

Falling downstairs backwards or forwards is dangerous and might be considered a life threat. Make sure your stairs are well lit, free from obstacles like toys or clothing, and has some sort of hand rail (which you USE!). Prevent babies and children from falling by installing a locking safety gate. When carrying items such as laundry and groceries, consider using reusable fabric-type grocery bags with handles so you can hold on to the railing with your other hand.

This can not be used 'instead' of chiropractic services, as it is often one component of chiropractic treatment. There is variety between different doctors of chiropractic, but total chiropractic care may include nutritional advice, exercise advice, mobilization, adjustments, physiotherapeutic modalities (laser, ultrasound, shockwave, etc), traction, postural advice, vitamin supplements, acupuncture, myofacial release (deep tissue massage), etc. Which of these treatment modalities are used is unique to each condition and/or patient.

Physical manipulation or tool Chiropractor?

As a chiropractor, I am of the opinion that there is no better professional to adjust the spine with appropriate purpose than a Doctor of Chiropractic. While physical manipulation may randomly help, an effective chiropractic adjustment moves vertebra in a specific direction to improve nervous system function, improve symmetry and biomechanics.

If your L5 disc is herniated do you need to have surgery?

Probably not.

Current research is suggesting that unless there are severe neurological signs (loss of bowel and/or bladder function), surgery is not a good option unless everything else has been tried and has failed.

Similarly, research suggests that imaging such as MRI is not a great indicator of the need for surgery or severness of a herniation, as as many as 30% of the general public with no back pain at all will have a "herniation" according to MRI (references 1-4 below).

Spinal manipulation by doctors of chiropractic has been shown to be very effective and perfectly safe for the treatment of herniated disks (references 5-7 below). Thus, it is always worth a try to see if a chiropractor can help.

Sometimes very severe disk problems can require a trip to a surgeon to ensure that neurological damage is not a risk. Your chiropractor will be able to inform you of whether or not you need to see a surgeon. Interestingly, a recent study found that 60% of people who were thought to need surgery for a disk herniation causing sciatica benefited equally as well from chiropractic care as they would have from surgery (reference 4 below). Thus, seeing a chiropractor for your disk herniation has a good chance of providing the same benefit as surgery in a majority of cases. A safe approach would be to try chiropractic, if it works (60% of the time) then you can avoid surgery, if it doesnt work (40% of the time) then the option for surgery is always still there.

This sounds like something that you read in an exam form or diagnostic report. ?What is being described is either active range of motion in the neck or passive range of motion in the neck. ?Active range of motion meaning how far you can flex or turn you neck yourself, passive range of motion meaning how far can the doctor move your neck. ?

Forward flexion meaning how far can your head move forward in degrees as you bring your chin to your chest. ?Extension meaning how far can you bring your head back like you are looking at the ceiling. ?Rotation meaning how far can you turn your head to the right or the left. ?Side ending, lateral bending, or lateral flexion meaning how far could you bend you head to the right or left side, as if you were bringing your ear toward your shoulder.

There are different "normal" values for active versus passive. ?And in fact there are different values for active or passive amongst many different textbooks. ?It sounds like "side bending" range of motion is reduced when bending to the right when compared to the left. ?It also sounds like "cervical rotation" is reduced when turning to the left when compared to the right.?

Reduced cervical range of motion is an indicator of some kind of neck dysfunction that is probably musculoskeletal. ?What that dysfunction is requires a complete history & examination that may include the addition of special tests like x-rays, MRI, CT, &/or EMG. ?You should seek the advice of your health-care professional to determine what this means in the context of a complete examination. ?From there you can discuss with your health-care professional how best to treat your problem.


Is it normal to have a stiff neck after going to the chiropractor?

Short-term muscle stiffness and/or soreness, nausea, headache and tiredness are actually quite common after a visit to the chiropractor. The latest research suggests that up to 50% of patients may have one or more of these side-effects for a short time following an adjustment. These side effects are more common in new patients and in patients that have gone a long time since their last visit. If you find that you have any of these effects after an adjustment you should expect them to last anywhere from 1-6 hours, and then should resolve. You should mention this to your chiropractic physician at your next visit so you and the doctor can discuss ways to prevent them in the future (eg: lighter adjustments, be adjusted more frequently, try a different form of therapy like mobilization or totally different like medication, etc.)

The answer is yes, a chiropractor can refer you to any health care provider or specialist that he or she feels is necessary based upon the history, the examination, & the progress with therapy.

If you are in severe pain while under the care of a chiropractor you should discuss with your chiropractor your options. If you feel you need medication while under chiropractic care, you need to discuss this with our doctor of chiropractic.

It is not uncommon for a chiropractic physician to want to try a short period of adjustments (joint manipulation) with physical therapy without medication to see if there is any improvement. In many cases this is effective and serves as an alternative to medical interventions. In some cases this is not effective enough, and a medical specialist has to be brought into the care to prescribe medication for the patient to take during their chiropractic care.

If your chiropractor is insistent that you do not need medicine, & yet you do feel that you do need pain medication, you may want to consider a second opinion. There are many different chiropractors out there that practice in many different ways. Some chiropractic physicians are very adamant against medicine, while others work very well within the medical paradigm. You may have to be diligent to find the best chiropractor out there that fits your needs.

Subluxation in medicine usually means a partial or incomplete dislocation. For example, if someones knee dislocates and then immediately goes back into place it is considered subluxed. Current recommendation for low back pain?

I am a physical therapist. Unfortunately as there is not one cause of low back pain there is not one "magic bullet" for back pain. One recommendation can be to start using ice. Apply ice to the low back for 10 to 15 minutes a few times a day. Avoid any activity that is aggravating. If the pain continues, got to a doctor how specializes in the low back. This way you will get an accurate diagnosis for your specific type of low back problem.

There are many recommendations available for the treatment of low back pain, dependent on who you talk to. The above poster is correct in that the optimal treatment usually varies from person to person as there are many causes of back pain, and everyone seems to respond individually to different therapies. The best recommendation I could give someone with low back pain that is not getting better as fast as one would like is to visit your local health care provider who specializes in back care (eg: doctor of chiropractic, doctor of osteopathy, doctor of physiatry, orthopedic surgeon, etc.).

A very thorough set of guidlines for the treatment of low back pain was released by the American College of Physicians in 2007 entitled: Diagnosis and Treatment of Low Back Pain: A Joint Clinical Practice Guideline from the American College of Physicians and the American Pain Society (Ann Intern Med. 2007;147:478-491). Anyone who reads these recommendations would note that there are many options for therapy, the key is to find one that works for you.

A very brief summary of the treatment recommendations of nonspecific or uncomplicated low back pain are:

Consider the use of drugs like NSAIDS in addition to the use of spinal manipulation, intensive interdisciplinary rehabilitation, exercise therapy, acupuncture, massage therapy, yoga, cognitive-behavioral therapy, or progressive relaxation.

A more comprehensive summary is:

It can be caused gradually or from a trauma. It's typically not seen in patients under 20 unless there was a trauma. In patients over 40, it's a rather common finding even in patients without symptoms.

Generally speaking, a goniometer is device used to measure angles. Goniometers are most frequently used in medicine to determine range of motion in a patient's joint. For instance, an injured person may lose some flexibility because of an accident. A goniometer could be used in such a case to help determine the extent of the damage. During rehabilitation, a goniometer would also be used to measure what progress has been achieved in returning back to normal range of motion.

Goniometers are cheaper and less accurate than inclinometers and other more technical instruments used for determining a patient's range of motion.
