What would cause acute ear pain while driving in a 2007 Nissan Sentra with the factory stereo turned on?

The most obvious (aside from loud volume) causes would either be a speaker problem or problem with the stereo itself sending a frequency signal that causes pain, or you may be sensitive to the stereo output frequencies due to the improved speaker system in that model year.

There's a couple ways to check:

1. Note whether it's all the time (any music, CD, MP3, radio, etc.) or just with certain music or stations.

2. Note the volume level it occurs at.

3. If it has a Mixer function, move all sliders to the lowest position then play music you know you've had the pain with. While it's playing, increase each slider all the way to the top until you feel the pain. Note which frequency ranges give you the problem.

If none of that helps narrow it down, then your next stop should be an ENT doctor for an ear exam. While certain frequencies cause pain, it's not normal for a car stereo to do that unless there's a problem with the unit or you. It's possible you may have an ear problem that's just now manifesting itself, but the only way to find out is by an exam.
