Not necessarily. People eat raw fish all the time - it's called sushi. However a good rule of thumb (if you aren't going to eat the raw fish as sushi) wash your hands after handling raw fish.

Why is it important to remove fish gills before cooking it?
I haven eaten plenty of small trout cooked over a campfire, and ate the head, gills, and bodies. With larger fish, it is probably well to remove the gills along with their guts so as not to disgust people.
