Aims of an emergency plan in an organization?

In an organization the key is properly prepared and reviewed "standing orders" for site response to various "situations". These orders will give the organization the ability to see it's limitations within it's scope of operation, should the need arise where other resources may be needed from other agencies, the organization could adapt in a timely manner.

Standing Orders are intended as a guide to "situation response", they are meant as a guide, in quick format with the built in ability to be tailored to fit various "situations" as no one "situation" is the same. An agreed protocol to how actions will taken during a "situation" that is understood by all responding personnel. Often response personnel are not "in the loop" with response protocols, this confusion can lead to the misuse and mis-deployment of man power and assets.

MMV stands for Mandatory Minute Ventilation. It is a derivative IMV (Intermittent Mandatory Ventilation). "with V" refers to the option to vent in this mode using volume as a set target rather than pressure.

The term "Mandatory Minute Ventilation" is just that; the user sets a minimum target minute volume (MV) by setting the rate (f) and Vt (tidal volume). The ventilator will then guarantee that the patient receives AT LEAST this predetermined MV. If the patient is doing no spontaneous breathing, the vent recognizes this and will provide full support for the patient, giving them that set MV. As the patient begins to breathe more on their own, the vent also recognizes this and will begin taking away mechanical breaths, allowing the patient to perform the work necessary to achieve the set MV.

Like SIMV w/ volume, in MMV, spontaneous breathing can be supported with pressure support (PS). UNLIKE SIMV, however (and as stated above), as the patient begins to breathe more on their own, the vent will provide fewer and fewer mechanical breaths providing that the patient is reaching at least the predetermined set MV.

This mode is a good choice for rapid weans, such as post-op patients, because as the patient begins to wake, the vent challenges the patient to maintain adequate ventilation on their own. The "mandatory MV" provides a safety net so that if the patient becomes more sleepy or begins to fatigue, the vent will recognize this and add support. This mode provides a less time consuming and controlled method of rapid weaning.

Important to note is that although this mode will guarantee that a minimum MV is reached, the vent user still must monitor the patient for signs of distress and respiratory fatigue. Spontaneous, rapid shallow breathing, for instance, may achieve your set MV, but achieving a MV with very small Vts and high rate is obviously not suitable for extubation.

Always be prepared. If you travel always carry at least a small survival kit, at least a warm coat, and if possible at least a small first aid kit. Also carry snacks in your car or boat, and extra water. Your survival kit should include something to start a fire with. Carry a compass, and map, if possible, and know how to use them. A coil of fishing line and a few fish hooks are good. A flash light or two are good with extra batteries. You need some kind of knife, perhaps on a multitool, an excellent item to have in a survival situation. It's also good to carry a signaling device, a signal mirror or a survival whistle. Carry a cell phone in case you find yourself stranded in an area where you have a signal. If you intend to be away from home for a while always tell a responsible person where you are going and when you should be expected back. It might be a good idea to carry at least a basic survival manual.

If you are stranded in your car stay there and make that your survival base camp, only deciding to travel and try to rescue yourself if it is clear no one will be looking for you anytime soon. If you have decided to try to rescue yourself and you find yourself in the wilderness, three fires are seen as a call for rescue.

If you have to take water from the environment, such as from a pond, it should be strained, and boiled for a maximum of ten minutes, a minimum of one minute.

Be up on plants in the area where you plan to go, so you'll know what you can safely consume.
