I believe you can re-open the claim up to 17 years. For links and information on Worker's Comp in Calif see http://www.steveshorr.com/workers.comp.htm
What is white collar syndrome?
As we know, white collars whose work usually do not involve manual labor but need to sit down for a long time in front of computers have been suffered kinds of pains including neck, shoulders, back, arms and wrists all over the world. The reasons of such pains, commonly, are muscle strain and blood flow reduction because they always keep the same posture. Why do the muscles become strained? Muscles require adequate blood flow through them to maintain performance. One way a muscle's blood flow is maintained is by the pumping action of contract-relax-contract-relax. In static working postures, the muscle just contracts and holds and holds and holds. The blood pumping action is missed and blood flow is reduced. People often try to use various methods such as heating pad, plaster, neck pillow, traction etc. to cure their pains. Such methods, to some extent, can release their pains but not stop them.

You can't really prove it, but you can have their test results disputed. If you have another independent test performed (you'd have to pay for it or get a lawyer to deal with it), and those test results dispute the others, you could then establish a legal challenge to their data.
Your best option is probably to get an independent analysis done, then if it disputes the other report, get a lawyer to sort it out. You should ALWAYS have a Lawyer when dealing with Comp issues or you will get screwed - period.
I am a physician who has previously contracted with One Call Medical. They never influence the results. One Call Medical makes the same amount of money no matter how the results turn out for you, they are a broker, they simply connect your employer's work comp carrier with doctors who perform EMG/NCS. However, there are other factors that could influence your results. One Call Medical pays less for their EMG/NCS than if the insurer contacted the testing physician directly. One Call Medical also typically does not pay for more elaborate testing and will pay the bare minimum number of tests. THose of us who are well trained in EMG/NCS often drop One Call Medical because they pay so poorly and will not pay for more elaborate EMG/NCS unless you write letters of need with long explanations. Sooooo, typically in the area I practice, the hungriest physicians, or the physicians that feel comfortable spending THE LEAST TIME performing the test are the ones that stick with One Call Medical. If you really want to know if your EMG/NCS was inaccurate, forget the test performed by One Call Medical and pay for your own test with a skilled and experienced examiner who is board certified to perform EMG/NCS testing specifically (there is a board certification for this test). Being board certified in neurology does not guarantee you are highly skilled at EMG/NCS. Also consider seeing a physiatrist, often these doctors conduct more detailed EMG/NCS.

Why is your hand itching after carpal tunnel surgery?
This is not too uncommon, sometimes the itching can be from the soaps used to clean your hand for surgery..it may take a week or or so but in some people it will cause a mild rash or itching. Also it could be from the skin being dried out or eczema. Also the skin in that area was kind of stretched so this sometimes irritates the nerves causing an itchy sensation later on. There's not a whole lot to be worried about and it should go away. Try soaking area in epson salts with warm water 2 x day, then apply neurtogena concentrated hand cream.
Does a limited liability company need workmans comp for one officer?
Your state may not require you to do so, but there are many benefits to carrying Comp, such as lifetime medical for a work-related injury, lost wages, retraining in a new job if you can't continue your old one, etc. If you're asking if you can be exempt from carrying the coverage, check with your state - and note that NO state releases you from liability, just from having to have insurance. Would the business survive if the limited liability company had to pay possible claims itself, while one worker couldn't work? Would the relationship survive?
The effective date normally means the initial or policy effective date in a reinstatement date of policy document. It can not mean reinstatement date solely. Technically it is not starting effective.
Can you file workmen's comp as income on your tax return?
You don't pay tax on workers compensation received by you or your survivors for job related sickness or injuries paid under a workers compensation act or workers compensation statute in the nature of a workers compensation act.
The tax exemption on your tax return does not apply to retirement plan benefits you receive based on age, length of service, or prior contributions to the plan, even though you retired because of an occupational sickness or injury.
If your employer continues to pay your regular salary or wages and requires you to turn over your workers compensation benefits you are taxed on your tax return on the overage that was paid to you by your employer.
The part of your workers compensation that reduces your social security benefits or equivalent railroad retirement benefits is considered social security benefits and may be taxable on your tax return under rules for those types of income. Accordingly, your workers compensation may be indirectly subject to tax on your tax return.
But, if your employer requires you to sign your checks over to them and continues to pay you, you will pay taxes as they will report the wages paid to you and the taxes withheld from those wages on your W-2. Some employers only supplement with accrued leave time with the employee receiving the worker's comp checks. That is the only sure way to have your WC benefit non taxable.

When you work on an honorarium you get payment for certain piece of work, without signing a labor contract. For example in the medical field, from a financial point of view, this type of employment is highly beneficial - for example - the hourly payment for doctors on honorarium is 180 euros on average. German clinics seek both assistants and specialists for work on honorarium which means that the stage of professional development is not an obstacle for taking advantage of this opportunity.
What are Workers compensation codes?
Workers compensation codes are classifications used by work comp insurance companies to classify the degree of workplace risk exposure for different jobs. A clerical office worker, for example, has a lower risk of being injured on the job than a firefighter or a roofer. The classification system is published and maintained by the National Council on Compensation Insurance, Inc. (NCCI) in a book called the Scopes of Basic Manual Classifications, and contains detailed descriptions of each classification. It is updated annually, and can be purchased from their website.
***** adding to the answer above******
Not all states follow NCCI Classifications. California for example has their own classification Manual. PA & DE, are completely different from NCCI codes, and have mostly 3 digit class codes, instead of the NCCI 4 digit codes. PA & DE also have a separate classification system for Staffing Employees. Although the above answer is true in a way, clerical employees can only be assigned to a clerical class code if and only if, they are in an office 100% of the time. A desk worker in a tiny room who works at a mechanic filing paperwork, that walks through the shop area multiple times a day, and doesnt have a separate exit, needs to be coded as the governing code of the Auto Body shop, and will not take a clerical code, even though their job is Clerical work. Misclassifications are typically the number one issue on a WC policy, and ive seen alot of insureds having to cough up the Additional premium amounts on the audit because they are unsure how to classify.

Does workers comp settle after a operation?
sometimes, it is usually decided when it
best benefit there own interest.
That sounds pretty dodgy - the Dr. should know better than to tell you the exact cause of your CTS was smoking. Go of your own accord to another doctor not connected to your employer insurance workers compensation to get another opinion - and ask the doctor to give you a written, signed and dated opinion. The insurance should be sending you to an unbiased 3rd party for evaluation. If it comes down to legal issues you'll be wanting a competing professional opinion anyways.
The problem with carpal tunnel is that in most cases its idiopathic - arising spontaneusly or from a cause that can't be readily established. While it has been found that very repetitive tasks are associated with higher cases of CTS, the issue is that it hasn't been established that they causes CTS (As far as I'm aware) - sort of like you know that people that eat mcdonalds are also likely to eat burger king, but they don't eat at burger king as a consequence of eating mcdonalds. The biggest risk factors for CTS are genetics or biological actors (such as smoking), rather than environmental factors or activity. However, that doesn't mean its due to smoking. It may be due to smoking, or genetics, illness (ie. arthritis) or tissue damage caused by 30 years of doing data entry.
If you're going after that it was due to the damage caused, ask the Dr. if it is due to a repetitive stress injury, if it may show up on a CT or Xray (or MRI?). They may refer you to get some, which might help support your argument.
