Because it deals primarily with bone structure, I would think it is used in medically-oriented literature. For example, the word is somewhat synonymous with foxhole. But it is unlikely that a writer would describe soldiers scrambling out of their lacunae.

On the third day? And we have been told nothing of the conversations or events that took place while a father was leading his only son to slaughter. Surely our imagination, filling in those lacunae, is more powerful than anything the author might have said.

Why is a by phrase frequently a sign of passive voice?

Let's look at a very famous active sentence.

Shakespeare wrote Hamlet.

Now let's put that into the passive.

Hamlet was written by Shakespeare.

So why do we use the 'by Shakespeare'. Well, read the sentence without it and it makes no sense. Also, ask yourself why we would have used that sentence in the first place.

Teacher: Who wrote Hamlet, class?

Freddy: Bacon, miss.

Teacher: Nice try Freddy, but Hamlet was written by Shakespeare.

Often though there is no 'by' clause.

John: Why so glum, matey.

Fred: My car was stole last night.

Fred wouldn't say 'My car was stolen by someone' because 'by someone' sounds silly.

So, in answer to your question, the by phrase is used in the passive voice when it is needed. Whether it is used (by people) more frequently, I don't know. (Did you see what I did in the last sentence?)

When deciding which to use, consider that the one that comes first might be seen to predominate. So maybe ?! is more like a question, and !? is more like an exclamation. But that is a very subtle difference and I doubt many people would notice or care.

BTW exclamation points are not much used in non-fiction, and their use in fiction should be sparing, unless you are writing a comic book.

I always use two spaces because that is how I was taught many years ago, but WikiAnswers always shortens it to one space when I send my answer. See? They just did it again. I think two spaces helps to separate the sentences, but perhaps it is no longer observed anymore, and one space is considered enough in the Digital Age.

i cannot show u how i write with my left hand but see i am typing with my left hand!! == When you learned to print, the result was shaky at best. You had to practice and practice to get the words to look right. The left hander had the same problem, but he learned to do it with his left hand. If you wish to learn, go get a first grade tablet and remember how you learned to write in the first place. Practice printing with your left hand, then work with script. Eventually you will retrain your brain and body to write with both hands. Next up...learning to spell!

Cornell note-taking is VERY hard, unless you are a person with MANY questions, and have the ability to not copy off someone on the Internet. (I did that a few minutes ago)
