It depends on the lab...most people believe that ANY lab can take any fluid you give them and run it through a little machine and anything the doctor wants to know/test for can be...its just not how it works.
There are stat labs that only run very basic stat/urgent tests for ER testing....and then all the way up to highly specialized labs that only do one particular type of testing: for instances TOXICOLOGY LABS or employee drug testing/ driving drug testing facilities.
In my lab we used a very basic (buy at RITEAID) kind of drug SCREEN that was used to immediately give the ER doctors an idea of what a comatose/unresponsive person MIGHT have in their system- so they could treat a possible overdose without being able to ask them what they took. These SCREENS are only supposed to be used to give the doctor an IDEA of what MIGHT be in the urine so they can treat....unfortunately they are also used by probation officers and methadone clinics, and run by people that have absolutely no idea how the screen works and that it is very VERY possible to get a positive when the person has ingested nothing illegal. These screens use the same basic technology as a pregnancy test and that is why you can now buy them online and in drug stores.
We had to increase our technology with the invasion of law enforcement into pain management doctors. All of a sudden our testing meant the difference between someone getting kicked out of medical care and we were looking for POSITIVE results rather than negative....because doctors started to kick any patient that came up negative for opiates, even though they were taking pain killers....or the these patients coming up positive for other drugs on our screen even though they didn't take anything but what they were prescribed. It was a MESS. Doctors didn't understand the technology and many of our clinical technologists (who were trained in clinical lab science, not toxicology) didn't understand the testing either.
As a result our lab now uses a half million dollar chemistry analyzer (that does a whole bunch of other testing besides drug testing) to run the drug screens....it is still a SCREEN, but it rarely gives us a false positive. AND we confirm all positive test in a TOXICOLOGY laboratory that actually does drug testing that actually (sort of) pulls the drug out of the urine and compares it to the actual drug....it's sort of like DNA testing for drugs.
This testing is highly specialized and accurate..gas chomatography/mass spectometry....but always remember that any lab test is only as accurate as the person running the test and human error IS possible.....we've all heard the stories of patients getting the wrong blood type or having the wrong leg amputated....so its not out of the realm of possiblity that a toxicologist could switch specimens/put the wrong label on (or even the person that just opens and labels the specimen) by mistake.
This is why it is my personal opinion, as a lab professional and an AMERICAN, that drug testing should not be used unless absolutely nessecary in ANY situation....a persons world can fall apart overnight (loss of job, license, criminal charges, family trouble) because of a false positive. If I truly believed that drug testing made it so that people with addiction problems could get the help they need, this would be different...however, drug testing is mostly used to "catch" someone and ruin their lives by incaceration or termination--it leads to sick people being turned away when they need help-I have never seen a person come into the ER and get a positive result on their drug test and have it lead to them changing their lives or getting medical care--it is used instead to DENY medical care and send people back to desperation/illn...which is NOT what why I wanted to be a medical professional...I want to help heal, not BREAK.
