The easiest way is to create playlists in MM to capture your song ratings, such as "All five star" import the playlist into itunes then set the rating the same. Same thing for each rating. Then you can create playlists based on any of the MM criteria you used.

I know of no way to capture the "number times played" and "lasy played" info from MM, so you would have to start over for that info.
