You didn't say what motor size. If it's the 3.9... check the air switching relay and the canister purge solenoid. (TBI systems). There are three solenoids located together just to the left and slightly behind the carb. The Air switching solenoid is in the center and the canister purge solenoid is to the rear. Also check the charcoal fuel canister for a broken fitting (replace it), and DOUBLE CHECK the fuel tank fittings and fuel lines for a broken fitting or sucking air (most likely the cause. I just solved my problem with the same issue. Had a broke fitting on top of the tank, on the vapor canister and a bad solenoid. Took forever to start the truck, ran rough, smelled raw gas and after about 3 weeks accidentally noticed fuel running out of my tank when I left it idle to recharge the battery.

I had a 1990 W150 318, so it should be in the same place. It is on the back side of the drivers side exhaust manifold. Your pickup is 2WD and should be easier then mine was. The axle and frame rail were both in my way, you may just half to deal with the frame rail and cross member. Good luck, they are a bear to break loose.

If you have the quad 4 engine beware. GM has something like 20 head gasket revisions. The head should be checked for flatness. DO NOT do the job without a machine shop checking this. New head bolts should be used. Talk to your local machine shop about the most recent torque specifications.
