I'm no doctor but I think its possible , the egg could start to split and grow but just like the way you can be pregnant with 2 babies without knowing but one disappears I think it could be that the egg started out as one and it gradually kept spliting if you get what I'm saying after all only one is a week older than the other and that's good now if it was a month that would be weird , anyways I've never been pregnant or anything like that but from where I stand I hope this helps you in understanding .

Today being the 6th of September 2009, then 77 weeks ago was the 16th of March 2008. 77 weeks is 539 days ago. You can use lots of things, like a computer, to take 539 days from the current date. You can do it in a spreadsheet. If you put today's date in one cell, say cell A1, then in another cell you can put a formula to subtract 539 days from it and it will give the result as a date, like this:

A 9mm gestational sac should correlate to between 5 or 6 weeks gestational age, by which time one should be able to see a yolk sac developing, although maybe not a fetal pole. It is true, that a small gestational sac may indicate very early pregnancy, and a repeat ultrasound and Beta-HCG level should be done. 2 weeks is probably a bit long to wait, however. I would repeat the HCG in 3 days, and another ultrasound in 1 week at the most.

While it is possible this is a miscarriage, you should wait for the repeat testing to be sure before you really get worried. Otherwise, you have spent this time worrying for nothing.

You did not indicate what week you were at in your question. I am at six weeks one day today, and went to my doctor today for an ultrasound. I had one twin with a fetal pole and a heartbeat that was about four times the size of the other one, which had the gestational and yolk sac but no detectable heartbeat. My doc said that there is a possibility that the smaller one will not be viable (that it may have arrested), but that a followup ultrasound in two weeks is necessary to see if there is growth. The ultrasound tech said that she has seen fraternal twins implant at different times, thus resulting in different size sacs and different rates of growth. Assuming you are not far along, I don't think the size itself or lack of a heartbeat is definitive of having lost one of them, and they will probably be able to tell at a repeat U/S. You can also look up the concept of a vanishing twin - it is apparently a common phenomenon. In most instances, it is not harmful to the remaining twin when it occurs early on.

The ultrasound doesnt give you the date. It is estimated by the days you give to the doctors to help, narrow down the date. The first day of your last period and how long you've missed help them in determining your conception date. The picture shows them an image of a fetus and they use that picture to say at what developmental stage the baby is in. Anything is possible.

I just wanted to add to the above answer. A patient's Last mentrual period is accurate if the patient has a regular mestrual cycle, but this is not the case for many women. Therefore, it is not the most accurate source for fetal age. Ultrasound is the most accurate dating tool for fetal age. The measurement of the embryo (crown rump length) up to 12 weeks gestation is the most accurate. The rate of growth of a fetus will be based on that early measurement for the remainder of the pregnancy. It allows physicians to monitor the growth of the fetus which in turn gives them an estimated due date as well as help them diagnose growth abnormalities such as IUGR (Intrauteriune growth restriction).
