Often they can be removed with your fingers but sometimes very gentle wiggling with a small needle nose pliers is necessary. If you grab it too hard with the pliers it can break the plastic. Replacement fuses are available at any auto parts store and are cheap.

One final note: If the fuse is broken you can see it by looking through the plastic with a light behind....there is a thin strip of metal that usually curves around inside the plastic and if its broken and maybe blackened in there the fuse is bad. If the fuse is bad you may also have a bad lighter as something happened to short out the fuse. Try replacing a bad fuse with ONLY a fuse of like amps and if it blows again you probably need to buy a replacement lighter.....these are also inexpensive. Sorry for being so longwinded about such a simple thing but....there you are.
