The transfer rate is great and since it is via a USB port, there is no air time as opposed to sending a picture via email.

Not any more than a Bluetooth ear piece would. The phone and car communicate through the Bluetooth connection, but the cars speakers are powered by the car battery, not the phone. The phone and car may use a small amount of power to maintain the connection when you aren't calling anyone, but that shouldn't be anything noticeable.

As to whether a Bluetooth call uses more energy on the cell phones part than using the phone's speaker, I believe it does slightly, but I am not sure.
