After replacing many parts (O2 sensor, fuel filter, plugs, wires, etc.) the problem was eventually found to be a faulty fuel sending unit. Expensive job which I couldn't do myself since it's inside the fuel tank and the tank has to be dropped to get to it. Once replaced, it's run like a champ ever since.
