The thermostat could be stuck closed. the heater core could also be plugged or the door closes when you use ac and opens when you use the heater may not operate.

I hate to say it, but if you have steam coming from the exaust, it's probably indicative of engine damage, possibly a blown head gasket. Generally if the radiator is cold, and the car is overheating, the possibilities include, a stuck closed thermostat, a stopped up radiator, a non-fuctioning water pump, or possibly a severly low level of coolant in the system, i.e., not enough coolant in the engine to circulate any through the radiator. Whatever you do, do not attempt to open the radiator cap while the engine is hot. You will severly burn yourself. Wait until the car is cold, open the radiator cap, and check the coolant level. My suspicion is that you will not see any coolant, because it has all either leaked out, or been boiled out. If you find this, I reccommend you get some help properly filling the radiator. It OS not as easy as just putting water in, because the system, has to be "burped" that is, the air pockets must be purged with a process that involves constantly carefully removing the cap while the engine is running, and adding more coolant several times. This can be dangerous if you don't know exactlt what you are doing because you can severly burn yourself strongly suggest you get some help. After all this, you I suspect you will find that the engine is damaged, and needs fairly expensive repair.

Sounds like you could have one or more different faults:
* A faulty coolant water thermostat: it may be stuck "always open" so any hot water from the engine block goes mainly to the car's radiator and hardly any is being allowed to reach the car's cabin heater. That is a very common cause of this problem in any car that is more than 10 years old. It should not be a big or expensive job to replace a faulty thermostat.
* There could be some air trapped (known as an air lock) or some other kind of blockage in the heating system which is preventing hot water from flowing from the engine block into the car's cabin heater element. The hose between the engine block and the heater element needs to be inspected for a physical blockage or an air lock.
* The engine's head gasket has blown slightly, allowing a small leak of exhaust gas to build up in the engine coolant. If that is what has happened another tell-tale sign - apart from the cold heater - is that some engine oil may be visible floating on top of the coolant water if the radiator cap is removed when the engine is cold. Any car mechanic should be able to check your car's heating system for the above faults. They are fairly common in any make of older car, especially if the engine coolant/anti-freeze has never been flushed out and renewed throughout a long running life.
